Transverse discrepancies are among the most common yet frequently under-diagnosed orthodontic problems. Accurate assessment of the skeletal, dentoalveolar, and functional components is essential for establishing a correct diagnosis and selecting the most appropriate treatment approach.
This three-hour presentation will focus on the management of transverse problems using Angel Aligner, highlighting the biomechanical possibilities offered by both soft and hard materials. Key diagnostic principles will be reviewed, including clinical and digital evaluation of maxillary and mandibular width, asymmetries, functional shifts, and unilateral posterior crossbites.
Through a series of clinical cases ranging from children to adults, participants will explore different treatment approaches for dentoalveolar and skeletal expansion, arch coordination, torque control, and correction of dental compensations. The course will also discuss clinical indications, limitations, and treatment protocols for managing transverse discrepancies in both growing and adult patients.
The objective of this session is to provide clinicians with practical tools for diagnosis, treatment planning, and efficient correction of transverse problems with aligners, ultimately improving functional outcomes, long-term stability, and facial aesthetics.

Dr. Palma is an international speaker with a clinical and academic focus on aligner biomechanics and treatment of growing patients. She has authored numerous articles in international journals and is co-author of Aligners Technique in Orthodontics (2021) and author of Biomechanics with Aligners: Attachments, Protocols, Features (2024).
Dr. Palma has treated more than 2,000 aligner cases and achieved Top 1% provider status in the EMEA region. She has been in private practice in Ciudad Real, Spain, since 2005 and is an active member of leading national and international orthodontic societies.

Treatment Strategies Based on the Maxillary Midline
Achieving esthetic treatment outcomes requires harmony between the maxillary dental midline and the facial midline, making it one of the key objectives of orthodontic treatment.
In this presentation, a unilateral premolar extraction case with mandibular deviation treated using Angel Aligner will be presented to demonstrate a treatment strategy focused on the position of the maxillary dental midline relative to the facial midline. Particular emphasis will be placed on evaluating the maxillary dental midline using the facial midline as the reference and on treatment strategies for midline management in clear aligner therapy.
Through this clinical case, digital treatment setup was used to evaluate the relationship between the facial midline and the maxillary dental midline and to incorporate this information into treatment planning. Rather than simply matching the maxillary and mandibular dental midlines, this presentation introduces a treatment concept that prioritizes harmonizing the maxillary dental midline with the facial midline and subsequently managing the mandibular dental midline based on that position.
Attendees will gain practical insights into the diagnosis of midline discrepancies, digital treatment planning, and biomechanical considerations for effective midline management, with the goal of achieving predictable and esthetic treatment outcomes in clear aligner therapy.

Dr. Yasuhiko Akama received his DDS from Kyushu Dental University, Japan, in 2017 and earned his PhD in Dentistry from Kyushu Dental University Graduate School in 2022. Following his doctoral training, he completed a Clinical Fellowship in the Division of Orthodontics at Kyushu Dental University. In 2023, he became a Certified Orthodontist of the Japanese Orthodontic Society.
Dr. Akama is currently the Director of Watanabe Dental & Orthodontic Clinic, where he provides orthodontic treatment for both children and adults. His clinical interests include clear aligner therapy, fixed appliance treatment, and the management of functional issues in growing patients.

本講演では、顔面正中に対する上顎歯列正中の位置に着目し、エンジェルアライナーを用いて治療を行った、下顎骨偏位を伴う片側小臼歯抜歯症例を供覧します。アライナー矯正治療において、上顎歯列正中を顔面正中と調和させるとともに、その位置を基準として下顎歯列正中をコントロールする治療戦略について解説します。
本症例では、顔面正中を基準とした上顎歯列正中の評価を行い、デジタルセットアップを用いた治療計画に反映しました。単に上下顎歯列正中を一致させるのではなく、上顎歯列正中を顔面正中と調和させ、その位置を基準として下顎歯列正中を調整することの重要性を提示します。
本講演を通して、アライナー矯正における正中管理の診断、デジタルセットアップを用いた治療計画、およびバイオメカニクスについて、日常臨床に役立つ実践的な知見を共有します。

クリアアライナー治療では、装置の剛性がマルチブラケット装置と比較して低いことから、歯体移動や圧下などの力学的コントロールが課題となる。本症例では、過蓋咬合を伴う患者に対し、エンジェルアライナーと歯科矯正用アンカースクリューを併用し、上顎前歯の効率的な圧下を行った。治療に際しては、エンジェルアライナーのHigh Trim Line を選択することでアライナー辺縁部の剛性を高め、装置のたわみを抑制することにより、圧下力の伝達効率の向上を図った。さらに、エンジェルボタンと歯科矯正用アンカースクリューを組み合わせることで、前歯部への安定した垂直的矯正力を付与し、不要な副作用を最小限に抑えながら上顎前歯の圧下を達成した。その結果、効率的な咬合挙上と過蓋咬合の改善が得られ、良好な咬合関係を確立することができた。本発表では、治療経過を供覧するとともに、High Trim Lineによるアライナー剛性の向上の影響とエンジェルボタンと歯科矯正用アンカースクリューを併用したクリアアライナー治療における力学的特徴とその臨床的有用性について考察する。

Redefining the Limits of Clear Aligner Therapy: Non-Extraction Gummy Smile Correction via Mandibular Auto-Rotation, Impacted Canine Traction, and Skeletal Class III Open Bite
In recent years, the clear aligner market in Japan has expanded rapidly; however, it remains difficult for patients to judge treatment quality in advance. As Akerlof's "market for lemons" theory demonstrates, this information asymmetry is giving rise to adverse selection, whereby low-quality treatment tends to drive out high-quality treatment. The key to overcoming this structural problem lies in diagnostic expertise grounded in clinical specialisation, together with the appropriate selection of appliances tailored to each case, as evidenced through clinical outcomes.
This presentation will begin with an overview of current market conditions and evolving patient needs, followed by a review of the following three cases.
Case 1 – Non-extraction correction of a gummy smile via mandibular auto-rotation. A combination of fixed appliances and clear aligners was used to facilitate mandibular auto-rotation, achieving improvement without extraction.
Case 2 – Surgical exposure and orthodontic traction of an impacted maxillary canine. Following surgical exposure of an impacted maxillary canine, traction and alignment were achieved using clear aligners alone.
Case 3 – Skeletal Class III open bite. Occlusal correction was achieved using clear aligners alone.
Ultimately, it is diagnosis and treatment planning—not the appliance itself—that determine treatment success. This presentation will highlight both the standalone potential of clear aligner therapy and the effectiveness of combined wire mechanics, with a view to considering the future position of clear aligners in orthodontic treatment.

Dr. Akimasa Morisaki graduated from Fukuoka Dental College in 2015 and completed his clinical residency in orthodontics at the college's Medical and Dental General Hospital the following year. He then joined the Department of Orthodontics at Kanagawa Dental University in 2017, where he also enrolled in the postgraduate program, completing his PhD in Dentistry in 2020 with research on resin cement adhesion to enamel following orthodontic debonding.
He began his orthodontic career at Koishikawa Orthodontic Clinic in 2020, before co-founding Aoyama Omotesando Orthodontic Clinic in Tokyo in 2024, where he continues to practise alongside a part-time role at Koishikawa. In 2025, he was certified as a specialist orthodontist by the Japanese Orthodontic Society.
Dr. Morisaki holds memberships with the Japanese Orthodontic Society, the Japanese Lingual Orthodontic Society, and the Tokyo Orthodontic Society. His clinical practice covers both fixed appliance and clear aligner treatment, with particular attention to the finer details of finishing and long-term retention.

Dr. Yoshito Oda graduated from the School of Dentistry, Kyushu Dental University, a public university corporation, and completed clinical training in oral and maxillofacial surgery at Matsue City Hospital. He subsequently trained at FaceTalk Azabu-Juban Orthodontic Clinic before opening Aoyama Omotesando Orthodontic Clinic in 2024. In 2026, he also opened Tenjin Orthodontic Clinic in Tenjin, Fukuoka City.
Drawing on his background in oral and maxillofacial surgery, his clinical practice is dedicated to exploring non-surgical approaches for cases that would conventionally be considered first-line candidates for surgical orthodontic treatment—including those involving impacted teeth, gummy smiles, and skeletal mandibular prognathism. In response to the increasingly diverse chief complaints of patients in recent years, he is committed to diagnosis and treatment planning that carefully addresses each patient's individual concerns, incorporating techniques such as mandibular auto-rotation.
He obtained certification from the Japanese Academy of Gnathology and Occlusion (Certified Specialist No. 7051) in 2025, and certification from the European Lingual Orthodontic Society (ELSO) in 2026.

本講演では、市場の現状と患者ニーズの変化を概観したうえで、以下の3症例を供覧する。

In clear aligner treatment, achieving planned tooth movement remains clinically challenging in cases requiring extraction space closure following premolar extraction, as well as in non-extraction cases requiring molar distalization. Since introducing Angel Aligner in 2023, the presenter has come to recognize its distinctive characteristics in tooth movement patterns, attachment design, and staging concepts through the use of protocols developed to address these challenges.
Angel Aligner offers the A7 protocol for premolar extraction cases and the A8 protocol for molar distalization cases. The A7 protocol is characterised by an overcorrection design that anticipates the reactive forces associated with extraction space closure, enabling more predictable space closure. The A8 protocol, on the other hand, features Angel Aligner's proprietary distalization protocol, which allows for efficient molar distalization.
This presentation introduces two cases—one premolar extraction case (treated with the A7 protocol) and one molar distalization case (treated with the A8 protocol)—and details the clinical process from diagnosis and treatment planning, through setup design on iOrtho, to actual tooth movement progression and treatment outcomes.
Through these two cases, the presenter discusses the criteria for protocol selection, key considerations during treatment planning, and approaches to addressing issues that may arise during treatment, offering insights applicable to everyday clinical practice.

Dr. Shohei Komatsu is a Specialist Orthodontist based in Chiba, Japan. He graduated from the Nihon University School of Dentistry at Matsudo and completed his postgraduate orthodontic training at the Nihon University Graduate School of Dentistry at Matsudo. He currently serves as a part-time lecturer in the Department of Orthodontics at Nihon University School of Dentistry at Matsudo.
Dr. Komatsu is a board-certified orthodontist of the Japanese Orthodontic Society with extensive clinical experience in fixed appliance therapy, clear aligner treatment, lingual orthodontics, and digital orthodontic workflows. He is particularly interested in integrating digital technologies and efficient treatment systems into everyday orthodontic practice.
He is a founding member of the orthodontic study group “Mokuyo no Yoruni (Thursday Night)” an educational platform for orthodontic specialists in Japan. He has also contributed to the Journal of Aligner Orthodontics and is a co-author of The Aligner Ortho.
As an active clinician and speaker, Dr. Komatsu regularly lectures and is recognised for his practical, evidence-based approach and his commitment to advancing modern orthodontic care.

エンジェルアライナーには、小臼歯抜歯症例に対応したA7プロトコルと、大臼歯遠心移動に対応したA8プロトコルが用意されている。A7プロトコルの特徴は、抜歯スペースの閉鎖に伴い生じる反作用を見込んだオーバーコレクトの設計にあり、より予測性の高い抜歯スペース閉鎖を可能にしている。一方、A8プロトコルは、エンジェルアライナー独自の遠心移動プロトコルにより、効率的な遠心移動を実現する点に特徴がある。

Anterior open bite cases remain as one of the most challenging malocclusions to treat in children and adult patients alike. Due to their multifactorial aetiology, complex biomechanics, and historically high relapse rates. Traditional treatment approaches often rely on orthognathic surgery or fixed appliance mechanics, both of which may present limitations in terms of patient acceptance, aesthetics, and long-term stability.
This presentation outlines a comprehensive clinical protocol for the correction of anterior open bites utilizing Angel Pro aligner therapy in combination with advanced biomechanical strategies and skeletal anchorage through mini-plates. Emphasis will be placed on diagnosis, case selection, digital treatment planning, and the biomechanical principles underlying the wedge effect, posterior dentoalveolar intrusion and mandibular autorotation. The integration of mini-plate anchorage allows for predictable vertical control and efficient force application, while aligner therapy provides enhanced patient comfort, aesthetics, and treatment precision.
Through a series of clinical cases, participants will gain insight into treatment sequencing, appliance design, force systems, and retention strategies that contribute to successful and stable outcomes. After completing a year in Health Sciences, and later physiotherapy, Dr Mo Al-Dujaili qualified as a dentist in New Zealand. He spent the next four years working alongside his mentor and father in Sydney Australia, where he practised all aspects of general dentistry with a special focus on surgical procedures. Gravitating towards the complexity of orthodontics, Dr Mo returned to Dunedin to specialise, gaining his specialist degree – a Clinical Doctorate in Orthodontics from the University of Otago.

前歯部開咬は、小児・成人を問わず、現在でも治療が最も困難な不正咬合の一つです。その原因は多因子性であり、生体力学的なコントロールも複雑で、さらに後戻りのリスクが高いことが知られています。従来の治療法では、外科矯正治療や固定式矯正装置による治療が選択されることが多いものの、患者の受容性や審美性、さらには長期的な安定性の面で課題が残る場合があります。

Non-extraction treatment in adults involving distal movement of bimaxillary molars using anchor plates and clear aligners
Distalization of molars in both arches is essential for the non-extraction treatment of anterior crowding or bimaxillary protrusion. Existing literature indicates that the limit for molar distalization using clear aligners is approximately 2 mm, and that exceeding this amount requires the adjunctive use of TADs.
For cases requiring more than 3 mm of distalization, I have employed a hybrid approach: initially distalizing the molars using partial bracket system combined with orthodontic miniplates installed at the zygomatic buttress and the mandibular body, and then switching to clear aligners after creating spaces in the anterior teeth. This method is considered particularly effective for anterior crowding cases, as the distalization of molars can lead to spontaneous resolution of the crowding following the “driftodontic phenomenon”. Additionally, in cases involving open bite, this approach offers the advantage of enabling molar intrusion simultaneously with distalization.
This presentation will introduce clear aligner treatment utilising orthodontic miniplates as absolute anchorages.

Dr Sugawara was selected as a "Master Clinician" in the April 2025 issue of *JCO* magazine, becoming the first Asian orthodontist to receive this honour.

前歯部叢生や両顎前突の非抜歯治療においては両顎臼歯の遠心移動が必須である。これまでの文献によれば、クリアアライナーによる臼歯の遠心移動量は約２mmが限界で、それ以上の遠心移動量が必要な場合にはTADsの併用が必要と報告されている。演者は３mmを超える遠心移動が求められ症例については、最初に臼歯部の部分ブラケット装置と頬骨下稜と下顎骨体部にインストールしたミニプレートを用いて両顎臼歯の遠心移動を行い、前歯部に空隙を獲得した後にクリアアライナーに切り替えるハイブリッドな方法を試みてきた。特に、前歯部叢生症例の場合には、臼歯の遠心移動に伴って前歯部叢生の自然改善（driftdontics現象）が期待できることから、有効な方法と考えている。また、開咬を伴う症例においては遠心移動と同時に圧下が可能であるという利点もある。今回の講演においては、ミニプレートを絶対的固定源に利用したクリアアライナー治療を紹介する。

Complutense University of MadridおよびUniversity of Salamancaでは大学院教育に携わり、現在はアライナー矯正治療におけるバイオメカニクスと成長期患者の矯正治療を専門分野として、世界各国で講演活動を行っています。
国際学術誌に多数の論文を発表しているほか、『Aligners Technique in Orthodontics』（2021）の共同著者、および『Biomechanics with Aligners: Attachments, Protocols, Features』（2024）の著者です。

Dr. Akimasa Morisaki graduated from Fukuoka Dental College in 2015 and completed his clinical residency in orthodontics at the college's Medical and Dental General Hospital the following year. He then joined the Department of Orthodontics at Kanagawa Dental University in 2017, where he also enrolled in the postgraduate program, completing his PhD in Dentistry in 2020 with research on resin cement adhesion to enamel following orthodontic debonding.
He began his orthodontic career at Koishikawa Orthodontic Clinic in 2020, before co-founding Aoyama Omotesando Orthodontic Clinic in Tokyo in 2024, where he continues to practise alongside a part-time role at Koishikawa. In 2025, he was certified as a specialist orthodontist by the Japanese Orthodontic Society.
Dr. Morisaki holds memberships with the Japanese Orthodontic Society, the Japanese Lingual Orthodontic Society, and the Tokyo Orthodontic Society. His clinical practice covers both fixed appliance and clear aligner treatment, with particular attention to the finer details of finishing and long-term retention.

Dr. Junji Sugawara currently serves as the Director of the Sugawara Orthodontic Clinic in Sendai, Japan and as a Visiting Clinical Professor in the Department of Orthodontics at the University of Connecticut School of Dental Medicine. After graduating from Tohoku University in 1973, he served as a faculty member there for 33 years.
Dr Sugawara has been an active member of the E. H. Angle Society (North Atlantic Component) since 2004. His primary research and clinical focuses include the Skeletal Anchorage System (SAS)—which utilises titanium mini-plates as temporary anchorage devices—and the Sendai Surgery First (SSF) approach, which enables surgical orthodontics without the need for preoperative orthodontic treatment. He has lectured extensively on these topics across the United States, Europe, South America, Australia, the Middle East, and Asia.
Dr Sugawara was selected as a "Master Clinician" in the April 2025 issue of *JCO* magazine, becoming the first Asian orthodontist to receive this honour.

After completing a year in Health Sciences, and later physiotherapy, Dr Mo Al-Dujaili qualified as a dentist in New Zealand. He spent the next four years working alongside his mentor and father in Sydney Australia, where he practised all aspects of general dentistry with a special focus on surgical procedures. Gravitating towards the complexity of orthodontics, Dr Mo returned to Dunedin to specialise, gaining his specialist degree – a Clinical Doctorate in Orthodontics from the University of Otago.
During the three years of specialist training, he had the privilege of learning from the best. The combined clinical and academic acumen of professors, doctors and staff provided an invaluable experience. Mo had the opportunity to teach undergraduate dental students, publish and present significant research as well as clinical cases at both national and international forums including; the International Association of Dental Research, the Australasian Begg Society of Orthodontics, the European Orthodontic Society and the New Zealand Dental Association conferences.
A firm believer in maintaining high clinical standards, Mo successfully completed further examinations to become affiliated with the Royal College of Surgeons (Edinburgh) and the Royal Australasian College of Dental Surgeons (Sydney). Mo is currently a part-time senior lecturer at Auckland University of Technology, a Fellow of the World Federation of Orthodontists and also holds professional memberships with the New Zealand Association of Orthodontists, the New Zealand Dental Association and American Association of Orthodontists. For the past 8 years, he has also been involved in supervising and lecturing of Oral Health students at the Auckland University of Technology. Drawing on his background in oral and maxillofacial surgery, his clinical practice is dedicated to exploring non-surgical approaches for cases that would conventionally be considered first-line candidates for surgical orthodontic treatment—including those involving impacted teeth, gummy smiles, and skeletal mandibular prognathism. 2015年に福岡歯科大学歯学部歯学科を卒業後、同大学医科歯科総合病院矯正歯科にて臨床研修を修了。2017年に神奈川歯科大学歯学部歯科矯正学講座へ入局すると同時に同大学大学院へ進学し、2020年に博士課程を修了、博士（歯学）の学位を取得した。学位論文では、矯正治療後のディボンディングを考慮したエナメル質へのレジンセメント接着について研究を行った。
大学院修了後は、小石川矯正歯科クリニックをはじめ、都内有数の矯正歯科専門医院にて矯正臨床の研鑽を積み、2024年に青山表参道矯正歯科クリニックを開設。2026年には福岡市・天神に天神矯正歯科を開設した。
臨床においては、マウスピース型矯正装置、表側ワイヤー矯正、舌側矯正まで幅広い矯正治療に対応し、患者一人ひとりのE-lineや顔貌との調和、個性正常咬合、さらには長期安定性まで見据えた診断・治療を重視している。

千葉県柏市にて矯正歯科専門開業。2013年に日本大学松戸歯学部を卒業後、琉球大学医学部附属病院口腔外科、および中部徳洲会病院口腔外科での勤務を経て、日本大学大学院松戸歯学研究科（歯科矯正学専攻）に進学。2019年に同大学院を修了した後、2021年まで日本大学松戸歯学部附属病院の矯正歯科にて、矯正臨床の研鑽を積む。2020年には日本矯正歯科学会の認定医資格を取得。2024年からは、日本大学松戸歯学部歯科矯正学講座の兼任講師を務め、後進の育成にも力を注いでいる。
臨床においては、固定式装置を用いた治療、マウスピース型カスタムメイド矯正、舌側矯正、そしてデジタルテクノロジーを活用した矯正ワークフローにおいて深い経験を持つ。特にデジタル技術と効率的な治療システムを日々の臨床に統合することに尽力している。
